Job Description
We are seeking a compassionate and dedicated Physical Therapist to join our team! In this role, you will assist patients with injuries and chronic health conditions in regaining their mobility, managing pain, and enhancing their overall quality of life.

Key Responsibilities:

Provide therapy services aligned with each patient's individualized plan of care.
Observe patients during treatment sessions and report on their progress.
Act as a liaison, maintaining open communication with patients, their families, and our clinical team to ensure seamless care coordination.
Deliver hands-on care and provide education to patients and their families.
Document all visits using our point-of-care software, Kinnser.net, which offers 24/7 access for your convenience.
Familiarity with OASIS E and adherence to CMS guidelines is essential.
Additional Information:

Company car will be provided for travel to patient visits.
Competitive visit rate ranging from $100 to $130 per visit, with each visit lasting approximately 45 minutes.
This position is part-time and contract-based.
